Meanwhile I have received an answer from o2-rufnummernmitnahme@telefonica.com and it states that they received my request incomplete/broken (“Dieser ist leider unvollständig bei uns eingegangen.”). I have no idea what’s exactly wrong with my form and the don’t tell any details me either.
Also they advice me to submit another request using online form (“Bitte gehen Sie noch einmal online unter www.o2.de/mein-o2/rufnummermitnahme und erteilen uns einen neuen Auftrag.”), which I already did yesterday (27.07).
Also, I’ve just contacted my former provider and they say the haven’t received a request to transfer from O2 yet.
