On August 29th, my iPhone SE2 was stolen, and my SIM card was also stolen at the same time.
We contacted O2 Prepaid Hotline about this problem. Then, a charged person of O2 Prepaid Hotline asked me to write the O2 form 'Prepaid – Besitzerwechsel' and send it to the the following address:

O2 Prepaid Kundenbetreuung
90345 Nürnberg
However, even though I sent the O2 form to the address above no less than twice, the both seems to have not been delivered to the address. Is the address above really correct? I think it is wrong or changed. Please answer this question within today.
