I left Germany already, for many reasons, and one of them is because I have no more Job. I am living already in my new place so there is no reason to pay for more than I used the SIM card, so could you please verify If my contract is really finisshed If not just finish it all?

You need to submit this form to o2 together with the required documents . If it did not work by FAX then you will have to send it by post.

I tried to send it by fax and its not working, Im already far from Germany, so would be plausible a simple email with this infos

unfortunately it’s not possible via email. Another option beside a fax is to send us the documents via mail, sorry. 
Did you maybe try again sending the form by fax?
