Which form?
If you want to revoke/cancel a direct debit (SEPA-Mandat) for an o2 prepaid sim card, then contact the o2 prepaid customer care using the o2 prepaid customer chat: www.o2.de/chat
If you want to cancel (complete deactivation) the o2 prepaid sim, then use this form:
