But a worker in the o2 hotline told me to send the formular for cancelation to this email adress. Is there any other one i can send the formular to?

If you want to revoke/cancel a direct debit (SEPA-Mandat) for an o2 prepaid sim card, then contact the o2 prepaid customer care using the o2 prepaid customer chat: www.o2.de/chat

If you want to cancel (complete deactivation) the o2 prepaid sim, then use this form:

But where do i send the form and how!? Is there any email adress?

Note, however, that you cannot cancel the prepaid card on a specific date. The cancellation will be processed immediately and the SIM card will be deactivated.
