Contract cancellation as I have to leave Germany

Dear O2,

 

I learned that I have to leave Germany, unfortunately.

I found information on this forum regarding the contract cancellation procedure, but the links to the form to fill out do not work, eg:

Clicking returns a server error.

Could you please provide a working link?
