Transfer credit out to my bank account from my prepayed card

Hello, 

I'm leaving Germany now. I want to get money out from myO2 prepayed card. I filled and signed the document of 'prepaid-guthabenauszahlung'. Who shall I send to? is there email address? Or anywhere to upload it?
