PUK and password

hi

 

I have lost my PUK and phone is blocked - I also dont have my password to get into my 02 account (to see the PUK) and when I try to reset this it sends code to sms to phone whoch I cant access

 

can you help please

    Pin and Punk are also printed onto the Voucher, in which sim was delivered.
