Hello.. I have an O2 post paid contract since June 2013 and a DSL connection since June 2014.
Due to some unforeseen circumstances, I will be moving out of Germany by the end of Dec 2016. Could you please send me the necessary forms and the list of documents I need to provide to terminate both of those contracts.

For the DSL contract there is no needed form. Please send us a written cancellation including your deregistration from germany to the following address:

For the cancellation of your mobile contract you need to fill out the form that you find attached with my answer here.
In that form you also find the address where you have to send all the documents.
Please keep in mind that you also have to hand in the deregistration letter of your place of residence in Germany, a registration paper of your new country or a confirmation of your new employer, as well as an agreement letter that o2 is allowed to charge 3 monthly basic fees as a replacement.
Another information- o2 will charge the costs of your bank account. So, you could leave your bank account open a little longer, or you change the payment that you transfer the money by yourself.

Regarding the documents you listed, I cannot provide a registration certificate of my new country as I am moving back to my home country and we do not have this process there. I'll not be working soon which makes it not possible to get a letter from my new employer. Is there an alternative? Is it enough to provide an national id with my permanent address in it, issued by home country?
Also I would be definitely closing my bank account while I leave Germany. So could you then provide me the account details to make the payments myself. That will be super helpful.

Don't you have any written confirmation of your new landlord that confirms that you're going to live in some new apartment?

As you had suggested, I wrote to@o2_Support a week ago and haven't heard back from them till now. Could you please check for me what's happening. I hardly have few more days left here in Germany and I really hope I will be able to finish this termination process.
Regarding the documents, shouldn't a copy of my residence permit (along with the de-registration certificate)be good enough to confirm I am leaving Germany for good? I still do not understand why it is important for o2 to know where I'm moving!
