Deactivate number because of moving abroad

I have a contract with O2 that includes package as well. I want to keep my phone and terminate the number earlier than two years.

Off-course I'll pay for phone as I am moving out of Germany for long term.

    If you like to deactivate your contract to an earlier date because of moving abroad, please fill out the following document:

    In this document you find a request for some specific documents that we need from you.

    Yes, actually I have two questions.

    I don't have any apartment yet abroad, Can I share the address of the company where I'll go?
    Secondly, what is a notification of your new, foreign domicile?

    thanks for your reply.

    Yes, a written confirmation of your employer or landlord would be okay.

    When you move to another country, you need to registrate yourself in the new country and you get a written document for that.
