Hi, 

my number is 0XXXXX56, I have prepaid sim, I want to move to freenet service provider. I want to porting my number to that network. 

I need to ask 3 questions.

1. What is the network provider for me, on mobile its o2-de and I don’t see O2 provider in their list. I need to know from where I can identify my service provider name (e.g. complete name. )
2.  I want O2 to release my number for the new contract provider. 
3. remaining balance how can I get back as right now i have 30 euros inside my balance.

please guide so I can smoothly transfer my number

Telefonica Germany is the provider. You release your phone number in the chat, as I already wrote here in the post. You can have your balance paid out using this form.

Prepaid credit payout

What should I do with that form, where should I submit it after fulfilling it? Do I need to attach here or some email address? please guidance is required.
