I sent a letter to cancel my contract and have not heard anything back. I've noticed now when logging in that my contract has been auto-renewed. What can I do to fix this and cancel my contract? I am leaving Germany soon.

there is no cancellation in your data until now.
Do you have any kind of proof for it? Like for example the advice of delivery for the letter?
Like I said, until now there has no cancellation of you reached the company. That the contract got extended for 12 more months is also normal, when there is no cancellation until 3 months before the normal runtime end.
If you want to cancel the contract because of moving abroad you have to send the following document as well as the other forms that the company asks for (which are mentioned right in the document):

I'm not sure which documents I can send to support this form.
- A registration certificate of your present place of residence.
I'm no longer a resident in Germany and in the UK there is no such document. I do have a UK bank account at my UK address. Will a letter from my bank be enough?
- a notification of your new, foreign domicile alternatively a confirmation of your employer or landlord
I have moved into my family house so there is no contract with a landlord. I'm also self employed so have no employer. What do you suggest I provide for this?

as I see you already got an information of another customer support worker:
A certificate of the bank account will not be enough. Somewhere has to be mentioned that the UK is where you have your place of residence.
If you moved back to your parents house they need to write a confirmation that you did, because they are the owner.

If you are leaving Germany for good, we would certainly like to help you and terminate your contract to an earlier date. For that purpose we still require copies of the following documents:
- A deregistration certificate of your present place of residence in Germany
- A notification of your new, foreign domicile
- Alternatively to the registration certificate a confirmation of your new employer or landlord
- Your approval for us to charge you the basic fee of three months.
Just enter the details and sign the document and send it back together with a copy of the documents required via fax to 01805 571 766* or via letter to the following address:

Be informed that by now your contract is still active!
