How to cancel new contract within 14 days

I had applied for a DSL connection on 23rd May. But now I want to cancel the contract. As I know that the online contract can be cancelled within 14 days, but I am not able to find the right communication point to cancel the contract. Can someone please help?

    No, the form is not neccessary, the only important thing is: clear words and information. ☺️

    Name, Address, Order Number or things like this and your will to revoke.

    To be on the safe side i would use german for the important sentence:

    Hiermit widerrufe ich meine Bestellung (Auftragsnummer) vom 23.05.2017 fristgerecht.

      how can I check or be sure that my contract has been cancelled?

      I have not received any communiation from O2 after my email to cancel the contract.
