Transferring account balance

Hello.  I recently transferred my number to another operator.  My account disappeared and my balance disappeared along with it.  It looked like privatization of my funds.  I want to transfer the balance from my o2 account to my bank account.

I'm guessing it's about a prepaid credit?

You can request the payment of your balance using this form: Prepaid - Guthabenauszahlung

It has to be sent in to the adress in the top right corner.
