thanks for pointing out. So I have to print and send this form by post to the address written on top right corner of the form, correct?

1. Can I get back the post fee?
2. Can I go to some O2 shop and submit this form there?
3. Can O2 shop close my account if I go there?

thanks for posting your request regardind cancelling your prepaid contract. 
Sending a letter by post will be on your own costs. You can asked a o2 shop to fax your form but it is not guaranteed that every shop will do this. 
The o2 Shops will not be able to close down your account.
