Hi,

there is no document needed, you can write a simple letter with adress, custom number and wish. Also o2 need a offical document that you leave germany, normaly the log-off confirmation.

The letter you can sent by post to

Telefonica GmbH

Überseering 33

or sent by fax to 01805-880088.

Jogi, for a DSL cancellation there is no special form needed.

For a mobile contract cancellation there is one needed.

If I didn't miss an information while reading now there is not mentioned if it's a DSL or mobile concern.

So, @kyun‌, if your talking about your mobile contract you need to fill out the form that I attached to my answer here and send it to the company (address mentioned in it). With that form you also have to hand in the other documents that are mentioned on the first page of it. ☺️
