Moving abroad, but new tenant wants to take over my O2 account. How to transfer account to them?

I am moving out of Germany to the US at the end of November, and the person who is moving into my apartment after I leave is interested in taking over my O2 internet + festnetz account.  I'm sure there are some transfer forms that we both need to fill out, but does anyone know where I can find these?

Remember that the change cost 29,99 € and your router must be snt back bacause the person got an new router (the routernumber is personalized). Here you can see the prices (see point 5)
