I had received confirmation of cancellation of my contract with O2 on 12-March-2018 by email.
The email also said that I will be billed for the remainder of the contract until its deadline which is 26-March-2019. For this reason I had kept my Deutsche Bank account active while I left Germany for my home country.
Now I have received communication from Deutsche Bank that I need to close my bank account as I have not been in Germany for the last 6 months. Therefore, I cannot continue to pay O2 mobile phone bill from the end of October.
Can you please help me in cancelling the contract completely in the next few days ? I have paid money to O2 even though I'm not using the sim card for last 6 months already.
I have read through the O2 community and found this information below.

This one you have to fill out and hand in as well the documents that are mentioned on the first page of it:
- a deregistration certificate of your present place of residence
- a notification of your new, foreign domicile
alternatively a confirmation of your employer or landlord
- we need now is your approval for us to charge you the all-inclusive basic fee for 3 months.
** only issue is with what to fill for my contact address in Germany, I do not have any registered address in Germany now.
can I send these documents to some one here who can take care of the cancellation ?

the form you've already found via the link you provided is the correct one, by sending in this form as well as all of the required documents to the address in Nürnberg your cancellation will be taken care of.
The fact that you don't have a current German address is not a problem, just fill in the last German address you were registered at (according to your account, it also is still visible as your contact address).
Please make sure to provide an email address, as that is where we will send further documentation and info about remaining bills to be payed.
If your account is closed before the final payment you will receive a mail which will provide you the information about how to pay it directly.
