I am a customer of o2 Blue Basic since 04.11.2013. My contract has been automatically prolonged, but now I would like to cancel it. I have sent my notification of termination in " Kündigung vormerken" page, but could not reach your service by phone. Could you please send me the cancellation form so that I can send it via post?



Furthermore, could you please tell me is there any other documents I need to provide, and how much do I have to pay to terminate the contract?
