sim card but no phone

Hi, I have received my SIM card, but no phone.I have already sent the cancellation form to the address by post, but do I also need to send back the SIM card? I got the SIM card after sending the cancellation  form.Please send me a confirmation email saying that my contract is terminated.

    And which cancellation form do you mean? The contract is still active.
