I have an O2 contract bearing no. +49 176 xxxxxx72. As I am moving out for good on 29.10.2017. Please kindly help me with the terminating procedure. Previously I have tried many times in canceling my contract, unfortunately, did not help me. Please do the needful.

I will be leaving Germany on 29.Oct.2017. As per your request, I had sent the cancellation form in the post. But I got a reply saying the contract will be canceled in April 2018.
Why do I need to pay until April 2018, when I am not using any services?

if you would have proceeded as it is mentioned in the form, then it would have been okay. But you didn't send in any of the forms that are asked for, which are mentioned in the document that Frechnuss linked here:

Without those documents the contract will continue until your normal runtime end in April.
