I'm sorry, I think my answer is too late now.

You have to log in to your "Mein o2"-Account with your mobile phone number and password. Then you can do this procedure online. You will see the phone number while doing this.

I suppose you want to cancel your contract before the end of its term. In this case please forget the hints above. You should send in the following form: https://static2.o9.de/blob/12985648/v=2/Binary/kuendigung-umzug-ins-ausland-download.pdf and  attach pieces of confirmation that you really go abroad. What certificates are needed and how much of fees you will have to pay can be read in the form.
