Ask for remaining deposit refund

Oh, I used Chrome translator to translate German to English, but it massed up my messages!

What I wanted to tell is as follows:

I using o2 myPrepaid S which ends on September 17, 2019. But since I return to my home country (outside European country) on September 12, 2019, I'd like to ask refund to my remaining o2 deposit to my German bank account (N26) as soon as possible before I close my German bank account.

What should I do?

With the credit balance, the number including the associated services is disabled.
However, I can not promise you that the payment will be made before 12.09.2019.

After filling that document, where should I submit? Can I send it via email, or is it only possible to send it by post?
Can I submit it to O2 shop directly?

Also, if the payment cannot be made before September 12, 2019, can I ask to transfer remaining deposits to my bank account (outside European country)?

You can also send the document by fax to 01805 57 17 66.
The payment can only be made to an European bank account (IBAN).
