[ENGLISH] Contract cancellation

Hi,

I need help to understand how to cancel my current DSL contract.

I'm moving to another country next April, so I would like to cancel my current contract by March. I tried with the phone service but my limited German does not help me. I'm afraid of making a mistake if I don't fully understand the instructions. I would really appreciate any help here.

My product is O2 DSL Light (includes a Land phone line) and my contract ends in 03.09.2016.

I think I need to fill a form together and send it via post mail together with my Abmeldung from the Rathaus.

Could someone please send me that form? Which is the address I should use to send the letter?

Also, what I have to do with my old Alice modem?
